TERMS AND CONDITIONS: The applicant agrees to attend all rehearsals and adhere to conduct standards. The applicant understands that in order to participate on December 18 he must be passing all classes for that grading period, must not have any discipline issues resulting in DMC or suspension, and must be in compliance with the Student Code of Conduct.

The Fee of $100 does not guarantee me a spot as one of the 18 contestants competing for Mr. CyCreek.. If more than eighteen entry forms are received an election will be held to narrow down the contestants. Anyone who submits the $100 Entry Fee will appear in the opening number of the Mr. CyCreek Pageant and therefore will attend practices.

Checks are payable to SBC. All Entry Fees are Non Refundable.
